New patients – please review all information listed below before making your appointment.  Please print the forms listed under numbers 1 & 2, then fill out the requested information and bring them to your first appointment.  Form #3 is about your privacy regarding your health information; you may print this for your record, but you do not need to bring it in.  If you are coming in specifically for acupuncture, you will need to complete form #4 as well.

If you are unable to complete the forms before your appointment time,  please plan to arrive at least 20 minutes early to allow for traffic conditions and time to complete your forms. You should notify us ahead of time, so that we can have the forms ready for you upon your arrival. This will allow us to devote your entire appointment time to your care.
